Dear young film buff:

• Please tell us about you and your favourite film below.
• There are five boxes to write in:
  Box 1: About you.
  Box 2: The name of your favourite film.
  Box 3: What your favourite film is about (the story).
  Box 4: Why you like the film.
  Box 5: An adult’s signature.
• Only hand-written forms are allowed.
• Try to make your writing easy to read – it may help to ask someone else to read your work and help you with hard spellings.
• Send in your entry by 31st July 2008. If your entry is late the judges cannot read it.

Parents and Guardians please note:

• Only children who are aged up to and including 11 years old on July 10th 2008 may enter.
• Only one entry is allowed per child.
• The entry form must be signed by you in Box 5 below.
• Only films with the following certificates can be nominated: U, PG, 12, 12A.
• Please do not make any changes to the form (such as making the text boxes bigger).
• Please help your child understand how to nominate their film. It may help to talk with them about it first; however, the film must be nominated by the child in their own words and handwriting, without any corrections or amendments from an adult.
• Only hand-written forms are allowed. If for reasons of disability this is not possible, please attach a brief explanation.
• The winning film will only be shown at the Showroom Cinema at a date and time to be agreed in 2009.
• The competition is to celebrate the 10th Anniversary of Showcomotion. Ian Soutar, Arts & Entertainments Editor of The Sheffield Telegraph will judge the competition and the winner will be able to choose a film to be screened at the Showroom Cinema in 2009. All efforts will be made to screen the film at a convenient time for the winner. Films are subject to availability; sometimes a first-choice film might not be available. The winner will receive 50 free tickets for friends and family and a dedication in the programme. All entries will be awarded a free pair of Showroom Cinema tickets. The decision of the Showcomotion Director is final.
